Accessing Your Medical Records

We give every patient access to their medical information so they can make better healthcare decisions. We provide two easy ways to access medical records:

Patient Portal

  • If you need help accessing your account, please submit a Contact Form with your name, date of birth, and date of most recent visit.

Medical Records Request

  • Access your medical records by submitting a request through our secure partner, BHS Connect.
    • BHS Connect will provide a tracking number once your request has been submitted. Track the status of your request here
  • Third-party medical records requests: For worker’s compensation, attorneys, and insurance providers seeking records, please fax the request to 843-536-4003.

  • For additional help, please contact the location where you received care for further assistance.
